In order to avoid conficts with regularly scheduled classes, reservations for each semester will not be processed until after the semester begins. Please fill in every field of the upper section of this form. The more specific your information is, the more likely the processing of your request will proceed smoothly. If you reserve a room for a weekend, the Monday before that event, you must confirm that you will be using the room or rooms. Please call 392-1374 x7237 and ask for Room Scheduling. If you do not call to confirm room use, we will cancel your reservations and the rooms will not be opened for your use.

The minimum criteria for scheduling a during-term assembly exam for a course is a total enrollment of at least 300 hundred and at least two sections.
During-term assembly exams are scheduled for periods E2 and E3 (8:20 PM to 10:10 PM). You need to specify three proposed exam dates (and day of the week) for each scheduled assembly exam. Space is provided to allow scheduling of up to five different assembly exams, all for the section and course specified above, at once.
It's okay to leave non-applicable fields blank in the lower section of this form.
